Healthy Families Case Manager	YE-031-2	Responsible for providing assessment, linkage & advocacy services to children 0-7 at risk of emotional disturbance & their families who reside in Martinsville, Franklin, Henry & Patrick counties who are enrolled in the Healthy Families program. Develop & implement individual services plans, complete required documentation & comply with all regulations applicable to program services.
